Septimus Smith hates Dr Holmes, because he thinks he is “the repulsive brute, with the blood-red nostrils” (Woolf, p. 84)
Dr Holmes did not correctly diagnose Septimus’ medical condition. The patient could be suffering from stress, shock, depression or Post Trauma Stress Disorder (PTSD).
Septimus thought Dr Holmes did not understand him and whatever the doctor prescribe, it didn’t help him enough to get out of his mental and physical states.
Rezia, Septimus’ wife, is Italian in origin. She thinks well of Dr Holmes. He had invited her to tea in his home. He would introduce his wife and four young sons to her. Rezia’s opinion of Dr Holmes was that “Dr Holmes was such a kind man.” (Woolf, p. 84)
